How MADs Work
A MAD is worn during sleep and gently repositions the lower jaw forward, drawing the tongue and soft palate away from the throat. This enlarges the airway diameter, preventing the tissue collapse responsible for snoring and apnea events. The forward advancement is typically adjustable for fine-tuning.
Clinical Efficacy
MADs reduce the Apnea-Hypopnea Index by 50–75% in most patients. For mild to moderate OSA, success rates approach 70–85%, with complete resolution in roughly 50% of cases. Both the AASM and AADSM endorse oral appliance therapy as a first-line treatment for mild to moderate OSA.
Potential Side Effects
Most side effects are mild and temporary — jaw soreness, tooth sensitivity, excess saliva, and morning bite changes. Most patients adapt within 2–4 weeks. A small percentage may notice minor tooth movement with long-term wear, which is why regular dental monitoring is essential.
Types of Appliances
Over 100 FDA-cleared designs exist, including SomnoMed, ProSomnus, Herbst, and TAP appliances. They are either monobloc (rigid, one-piece) or titratable (two-piece, adjustable). Custom-fitted titratable devices made from dental impressions are strongly preferred over over-the-counter alternatives.
The Treatment Process
Treatment begins with a sleep study to diagnose and grade your OSA. Your dentist takes digital impressions, and a custom appliance is fabricated in 2–3 weeks. A titration period gradually advances the jaw to the optimal position. A follow-up sleep study confirms effectiveness.
Risks of Untreated OSA
Untreated OSA carries a 2–3× increased risk of cardiovascular disease, hypertension, Type 2 diabetes, and stroke. Cognitive impairment, depression, and elevated accident risk are also documented. Studies associate severe untreated OSA with significantly shortened life expectancy.
Who Is a Good Candidate?
MADs are most effective for adults with mild to moderate OSA (AHI 5–30/hour) and primary snoring. Importantly, MADs are also a recognized treatment option for patients with severe OSA who are CPAP intolerant — when CPAP cannot be used or tolerated, oral appliance therapy is clinically appropriate regardless of OSA severity. Most custom appliances last 3–5 years with proper care — daily rinsing, gentle brushing, and cleaning tablets. Insurance plans that cover oral appliances typically allow replacement every 3–5 years. Bring your device to regular dental checkups so your dentist can assess wear and effectiveness.
Medical Insurance Coverage for Oral Sleep Appliances
Appliances prescribed for diagnosed OSA are typically covered as a medical benefit — not dental — under most major plans including Medicare Part B. Coverage requires a physician-ordered sleep study and a letter of medical necessity. Important: Appliances for snoring alone (without an OSA diagnosis) are generally not covered. Many dental sleep practices have insurance coordinators on staff who handle pre-authorization and billing on your behalf.
